AGREEMENT TERMS
- All terms are final, membership fees are non refundable.
- Full payments will be accepted by Visa and Master Card and cash (Gold or Bronze Programs)service fee of 4% on total .
- Any (Silver Programs) payment plans with any payments set up to pay the balance of tuition owed are just that, they are scheduled payments on the total purchase agreement.
- You are responsible and I the undersigned agree to pay set scheduled payments regardless of attendance made to the course and the program.
- Please provide your void check or banking institute information for all (Silver Program) payment plans.
- All payment plans will be deducted from an personal checking or savings account paid by electronic funds transfer (EFT)

- This Agreement may be cancelled in a case of death, or if the school ceases operation at the location where you entered into this agreement.
- If you become disabled, you shall have the option of (1) being relieved of liability for payment on that portion of the agreement for which you are disabled by freezing your payments for the duration of disability.You must prove such disability by a doctor’s certificate, which shall be enclosed with the written notice of disability sent to
- TNT School Of Martial Arts (2) If after said disability you wish to cancel the agreement you may by paying a fee equal to two agreed payment on agreed scheduled payment plan.
- Any payments not received within two weeks (14 days) of the above collection date will result in a $25 Late Fee, which may be drawn automatically if applicable. An extended delay of tuition payment may result in further collection procedures and/or legal action.
- If a payment is returned there will be a $25 Non-Sufficient Funds Fee added on.
Release and Waiver of Liability
I, The undersigned on behalf of myself, members of my family, my heirs, executors, administrators and assigns, hereby forever release, discharge and hold harmless TNT School of Martial Arts/6548326 Canada Limited, representatives and agents for any injury, loss or damage to my person or property howsoever caused, arising out of or in connection with me taking part in martial art classes, self-defense seminars and activities and not withstanding that the same may have been contributed to our occasioned by the negligence of TNT School of Martial Arts/6548326 Canada Limited, representatives or agents. I also understand that video and photos are often taken and I release liability for any film and pictures taken, and are used in web page and social media platforms.
Please note:
participants must have their own uniforms and protective equipment.
This waiver was read and he/she agrees to abide by it.
